To set up automatic payments from your bank account, please complete and return the Automatic Bank Payment Form to us by fax at 888-255-2060, secure email or to our mailing address. If you have any questions, contact Customer Connections at 800-869-0355, Ext. 4279. Approximate timeframe to complete set up once the form is received is 4-5 business days.
Set up direct deposit

To set up direct deposit to your bank account, please complete and return the Direct Deposit Form to us by fax at 888-255-2060, secure email or to our mailing address. If you have any questions, contact Customer Connections at 800-869-0355, Ext. 4279.
Request a loan or withdrawal

To request a policy loan or withdrawal, please complete and return the Loan/Withdrawal Request form to us by fax at 888-255-2060, secure email or to our mailing address. It only takes about 5 business days to process your loan or withdrawal. If you have any questions, contact Customer Connections at 800-869-0355, Ext. 4279.
Get a copy of your policy or certificate
Download form and send in request.
If it’s your first time requesting a copy of your policy, it’s free – otherwise, there is a $20 fee. Complete and return the Duplicate Policy/Certificate Request form to us by fax at 888-255-2060, secure email or to our mailing address. It only takes about 2 weeks for you to receive the copy. If you only need a description of your policy benefits, contact Customer Connections at 800-869-0355, Ext. 4279 and we can get that information to you within 5-7 business days.
Change your beneficiaries
Download form and send in request
To make a change to your beneficiary(ies), please complete and return the Beneficiary Designation form to us by fax at 888-255-2060, secure email or to our mailing address. It only takes about 7-10 business days to process your request. Once the beneficiary designation has been updated on our records, you will receive a letter confirming that the beneficiary change has been completed.
Change of name or address

- Update your address by logging into your MyAssurity account; or
- Complete and return the Change of Name or Address form to us by fax at 888-255-2060, secure email or to our mailing address.

- Submit a government-issued document (driver’s license, marriage license, etc.) showing your updated name; or
- Complete and return the Change of Name or Address form to us by fax at 888-255-2060, secure email or to our mailing address.
