If you wish to report a death, initiate a claim or check on claim status, please contact us by any of these methods:

You may also fax a Notification of Death form to (888) 869-0368 or (402) 437-4592 (local).

Please be able to provide the following information when you contact us:

• Name of the insured individual
• Policy number
• Date of death/disability
• Your name and relationship to the insured
• Mailing address for claim papers to be sent
• Your daytime phone number and the best time to reach you
